Consent form

I understand that my jewellery will be welded closed and has no clasp, so it can only be removed by cutting it. The weld is made with a small welding tool that produces a brief spark at the joining ring, with a protective patch placed between the chain and my skin. I have told, or will tell, my Loop'd artist before the weld about any metal allergy or skin sensitivity I have, and about any pacemaker or other implanted electronic medical device. I will check the chain, length and placement with my artist before the weld is made. I understand that I may need to have the jewellery cut off for medical procedures, such as an MRI scan or surgery, or for some security checks, and that arranging this is my responsibility. I understand that, once my piece is fitted, Loopd Jewellery House Ltd is not responsible for loss of or damage to the jewellery caused by snagging, pulling, cutting or everyday wear, to the extent permitted by law.
